Referrals

Cambridge Nursing and Rehab Center accepts referrals from:

• physicians
• social workers
• nursses
• therapists
• discharge planners
• insurers, case managers
• patients themselves
• family members
• friends 

We establish tenative discharge dates at the time of admission and make sure referral sources are kept up to date of the resident's progress as well as aftercare plans.

Admission

Our Admission process is easy!

Just contact one of our admission staff and  We handle everything including assessment and paperwork.

Admitting Coordinators will conduct an on-site evaluation of prospective residents to identify thier care, rehabilitation needs, diet, etc.  We also provide personalized tours of our facility.

In providing our services, Cambridge Nursing and Rehab Center does not discriminate on the basis of sex, age, race, religion, national origin or disabling condition.  We accept third party payer types and we serve Medicaid recipients with disabilities.
