provide one-on-one interventions to assit residents
work to ensure that the residents' time is productive
offer support groups for residents
provide counseling and support
assist with discharge planning
provide transition services including: coordination for home health care and medical supplies and neccessary equipment.
If there is need for assistance at home, our counselors will contact the Home Care Agency and arrange for help with housework, cleanin, cooking and other household activities the resident may require.
Our residents and thier families are encouraged to express any questions they may have with our Social Services Counselors.
